From ICD-10 lookup to a note you have reviewed.
Keep diagnosis lookup and note drafting together, while retaining control over what goes into your final documentation.
1. Search a diagnosis name or code
Start in the ICD-10 search field. Enter a diagnosis name or a code, then review the matching entries. For a simple walkthrough, search for I10 or essential hypertension. This example illustrates the interface; it is not a patient assessment.
2. Select the relevant entries
Choose the diagnosis that matches your intended documentation. Use Add ICD10 when you need additional entries. Confirm the selected labels and codes before generating a draft.
3. Generate a starting point
Generate Note creates draft text from the selected diagnoses. A diagnosis label alone does not establish a patient’s history, examination, medication use or treatment plan. Treat the output as a starting point that requires your review.
4. Review, edit and copy
Use the editor to correct the text and remove anything unsupported. Dictation is available in English and Spanish. Copy the note only after checking that it accurately reflects the encounter. Saved notes for individual codes can be restored within your account.
Verify coding details when they matter
Search results are an aid to your workflow. Confirm the applicable ICD-10-CM release, specificity and coding instructions against authoritative references before using a code for reporting or billing. The CDC ICD-10-CM resources provide official background and access to classification materials.
